EMPLOYEE BENEFITS OVERVIEW:
Full-time employees of Number One Vending Service, Inc. are entitled to the following benefits after a 90 day probationary period.
Medical insurance is provided by Humana Health Insurance. The policy is a PPO that offers:

- Office Visit Co-Payment - $15 |
| |
- Prescriptions - $10 for generic, $20 for brand, $35 for non-preferred |

- Emergency Room Co-Payment - $50 |
Number One Vending Service, Inc. will pay 50% of the monthly premium for the employee, and the employee can put dependents on the policy at their own cost.
Dental Insurance is provide by Combined Insurance Company of America. You can use any dentist that accepts this insurance.

Annual Deductible - None for preventitive services, $50 per person on basic and major |

Preventive - paid at 100% |

Basic Procedures - paid at 80% |

Major Procedures - paid at 50% |
Number One Vending Service, Inc. will pay 50% of the monthly premium for the employee, and the employee can put dependents on the policy at their own cost.
Holidays - The employees of Number One Vending Service, Inc. receive up to six paid holidays per year that include:

Vacation - After one year of full time employment, employees accrue two weeks of paid vacation.
